The honest answer is that it depends enormously on technique — and one of the most important technical decisions happens before the first incision is ever made: where that incision is placed.
The Natural Crease: Nature's Perfect Hiding Place
Every upper eyelid has a natural crease — the fold where the eyelid skin tucks in when your eyes are open. It's one of the few places on the face where an incision can essentially disappear, because the crease itself conceals it every time your eyes are open.
In our practice, we place the incision precisely at the natural crease, or just above it. This accomplishes two things at once:
- The scar is hidden in plain sight. Once healed, the incision line sits within the fold of the eyelid, invisible in normal conversation, in photographs, and even up close. 2. The crease can actually be enhanced. Thoughtful placement doesn't just remove excess skin — it can define and refresh the appearance of the upper eyelid, restoring the crisp, open-eyed look that heavy skin has hidden for years.
Why Placement Is Not One-Size-Fits-All
Crease height varies from person to person and between ethnic backgrounds. A well-planned blepharoplasty respects your anatomy rather than imposing a standard template. Before surgery, we carefully mark the crease with your eyes open and closed, measure the skin, and design the incision so that the final result looks like a younger version of you — not a different person.
What Poor Placement Looks Like
When incisions are placed too high, too low, or asymmetrically, the results can be hard to correct:
- A visible scar above the fold that shows when the eyes are open
- Asymmetric creases between the two eyes
- A "hollowed" or "surprised" look that reads as operated-on
This is why upper blepharoplasty — despite being a relatively short outpatient procedure — rewards experience and meticulous planning.
