
Not all upper eyelid surgeries are performed the same way. One of the biggest differences between techniques — and one most patients have never heard of — is what happens to a thin, delicate muscle called the orbicularis oculi.
Meet the Muscle That Blinks for You
The orbicularis oculi is the circular muscle that surrounds your eye. Every time you blink — roughly 15,000 to 20,000 times a day — this muscle is doing the work. Blinking isn't just a reflex; it's how your eye spreads tears across its surface, keeping the cornea moist, comfortable, and protected.
In traditional blepharoplasty, many surgeons remove a strip of this muscle along with the excess skin. In our practice, we take a deliberately different approach: we remove only the redundant skin and completely preserve the orbicularis muscle.
Why We Believe Muscle Preservation Matters
In our experience, sparing the orbicularis offers several meaningful advantages:
- Natural blinking is preserved
Because the muscle is left fully intact, its function — the strength and coordination of your blink — is undisturbed. Your eyelids continue to close completely and comfortably, just as they did before surgery.
- Sensory nerves are protected
Fine sensory nerve branches travel within and beneath the orbicularis. Removing muscle can injure these nerves and leave patients with numbness along the lash line and eyelid margin — an underappreciated but frustrating outcome. By staying in the skin-only plane, we help protect that sensation.
- Motor nerves stay out of harm's way
The tiny motor nerve branches that power the eyelid's closure are likewise shielded when the muscle layer is never violated.
- It may help reduce the risk of dry eye
Dry eye symptoms are among the most common complaints after eyelid surgery. Because an intact orbicularis supports a full, effective blink — the pumping mechanism that distributes and drains tears — we believe muscle preservation may help lower the risk of postoperative dryness and irritation.
Does Skin-Only Removal Still Give a Great Result?
Yes. For the vast majority of patients, the heaviness, hooding, and tired appearance of the upper eyelids come from excess skin — not muscle. Removing that redundant skin while respecting the deeper structures delivers a refreshed, brighter eye while keeping everything that works, working.
